Office visits:
Payment required at time of service or by prior arrangement with office manager.
Laboratory testing and urine drug screens:
If Urine drug screen or oral swab done by LabCorp, ADL or other outside contract labs, patient or his/her insurance will be responsible for costs.
All blood-work must be done by outside laboratory. We can provide lab locations for your convenience.
Additional screenings or labs: information on costs available from office staff.
*** If patient insurance not in our panel of providers, patient will be responsible for payment. Office will assist as needed with claims to insurance provider.
We accept credit cards, debit cards, money orders, personal checks. No cash transactions in our office. No paper prescriptions in our office.
